Community Liaison

Full-time

SUMMARY:

The Community Liaison works under the supervision of the Director of Business Development and is responsible for increasing public awareness of the agency, increasing active patient/client count, developing referral sources, representing the agency in community events and coordinating with clinical staff to identify needed services for patients in all stages of life. Provides direct education regarding hospice services and supportive services to referral sources, physicians, and the community. Serves as a resource for staff. Performs other duties as assigned.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Community Awareness

  • Increases public awareness of the agency and the services provided.
  • Makes contact in–person and via phone to the target population (i.e. hospitals, extended care facilities, physician offices, rehabilitation centers, community organizations, etc.) according to the specified frequency.
  • Assists with advertising according to budgetary guidelines.
  • Represents the agency at community events, fairs, and expos
  • Assists with the development and implementation of agency sponsored events.

Physician Relationships

  • Develops and maintains referral contact lists.
  • Documents weekly contacts and follow-ups.
  • Reviews potential referrals for additional service opportunities and recommends services

Marketing

  • Creates personal marketing program for assigned territory.
  • Increases active client count, hours of home care services and number of patient referrals. Must be comfortable having a hospice conversation with family’s and patients.
  • Educates referral sources, patients and/or families regarding agency services available.
  • Meets or exceeds departmental productivity standards.

Education

  • Educates the general public on agency services via presentations, visits to organizations, group meetings and direct mailings.
  • Conducts meetings to educate referral sources on changes in Medicare/Medicaid standards that impact referrals to hospice and supportive services.
  • Assists in the development, implementation, revision and evaluation of departmental policy and procedures.
  • Notifies staff in writing of new community resources and updates the community resource book as new information becomes available.

Education and/or Experience

  • Associates or bachelor’s degree preferred.

Two years marketing experience preferred in hospice. Track record of success encompassing customer service, community relations, and physician relations preferred.

Palliative Care/Hospice Nurse Practitioner

PRN with availability to grow into Full-time

As a Nurse Practitioner on the Palliative and Hospice Care Team, you will provide Palliative and Hospice Care to patients in the hospital setting and/or skilled nursing facility and/or home setting. This excellent opportunity affords a flexible, autonomous role bringing enormous satisfaction in the care and comfort of our aging population. Additionally you will build meaningful relationships with the residents and their families.

Our nurse Practitioners collaborate with the Executive Director, Medical Director and Hospice Clinical Director to ensure that quality Palliative and Hospice Care is being provided to our patients.

Required Qualifications:

  • Current, unrestricted NP license in the State of IN
  • A clinical background in adult, family or geriatric specialties
  • DEA or ability to obtain
  • Prescriptive authority or ability to obtain
  • Minimum 1 year NP experience
  • Ability to work autonomously
  • Reliable transportation and insurance
  • Ability to travel throughout our service area
  • Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook

SEEKING PRN MIDNIGHT HOSPICE RN's or LPN's

TO WORK IN OUR 13-BED INPATIENT FACILITY

 

POSITION SUMMARY

The Registered Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.

The Licensed Practical Nurse performs designated nursing functions under the direction of the Registered Nurse. The activities of the LPN are based on his/her educational preparation and experience.

MAJOR FUNCTIONS:

• Assumes primary responsibility for nursing care of the patient in conjunction with physician’s orders and IDT.

• Understands and can interpret to patient and family the philosophy, acceptance, admission, recertification, and discharge policies for all agency programs and levels of care.

• Observes, evaluates, and reports to physician patient’s physical and emotional condition, reaction to drugs and/or treatments. Interprets to physician social and physical factors in the environment that affect patient’s care.

• Identifies nursing problems and establishes plan of care based on mutual goals.

• Makes ongoing re-assessment of patient/family’s current status and initiates changes in plan of care as necessary.

• Evaluates need for additional agency and/or community resources and makes appropriate referrals including caregivers at risk for pathological grief.

• Coordinates and/or implements other agency professional services for his/her patients and families. Prioritizes needs with the members of IDT.

• Instructs and supervises HHA/LPN’s.

• Consults with and educates the patient and family regarding the disease process, self-care techniques, prevention strategies, end of life care and process to deal with issues of ethical concern.

• Helps patient and family to adjust positively to physical, mental and social limitations. Helps family accept and cope with responsibilities for providing care.

• Respects the right of the patients to maintain reasonable control of their lives and retain as much decision-making authority as possible.

• Helps to achieve and maintain continuity of patient care by planning and exchanging information with other health and social agencies as well as by intra-agency exchange of information.

• Utilizes the team approach to achieve and maintain continuity and comprehensiveness of patient/family care.

• Serves as an auxiliary member of IDT and attends IDT meetings when requested to do so. Participates in re-certification process per agency policy.

• Abides by shift schedule. Performs other duties as assigned.

• Provides specialized hospice training to other staff, family members, and informal caregivers to insure adequate care.


QUALIFICATIONS:

• Must be currently licensed as a registered professional nurse or Licensed Practical Nurse

• Graduate of a state accredited school

• Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or home care. Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred. Demonstrates comprehensive knowledge and skill in current nursing practice.

• Must hold and maintain a valid driver’s license, auto insurance.

Hospice Social Worker

The Visiting Nurse Association of NWI is seeking a Hospice Social Worker (full-time) to provide direct supportive services to terminally ill patients and their families at our in-patient Hospice Center and Patient Homes. The Hospice Social Worker works in conjunction with the Hospice interdisciplinary group (IDT) to provide direct supportive services to terminally ill patients and their families, and to consult with and support other agency personnel. The social worker will provide assessments of psychosocial problems and define goals and interventions to provide excellent end of life care.

 

The Hospice Social Worker:

· Is a member of an interdisciplinary team (IDT) and provides social services in accordance with interdisciplinary plan of treatment

· Participates in the preparation, evaluation, and execution of the interdisciplinary plan of care to assure that the highest quality of care is given to those served

· Recognizes and utilizes community and family resources to assist the patient and family with end of life care needs

· Develops the plan of care with the IDT to deal with personal, financial, and environmental difficulties that may interfere with care

· Assists with funeral planning

· Keeps abreast of social work trends and knowledge

· Participates in education both internally and externally to promote professional growth and understanding of hospice

· Maintains continuity of patient care by liaising with other health care professionals

· Attends IDT conferences and care plan meetings

· Arranges for extended care placement or respite placement when necessary

Job Requirements

· Demonstrates flexibility with scheduling

· Must possess a Master's Degree from a school of social work accredited by the Council on Social Work Education

Required experience:

· Minimum of one year in social work, preferably in a healthcare related setting
